Children who are missing their upper lateral incisors must wait until they are finished growing to have dental implants placed. Dr. Jennifer Eisenhuth maintains space in the area until they are ready for the implants.

EAGEN, MENDOTA HEIGHTS, ST. PAUL AND MINNEAPOLIS, MINNESOTA – Lots of kids want their space by the time they reach their teenage years, and Mendota Heights orthodontist Dr. Jennifer Eisenhuth is giving it to many of her patients, in a manner of speaking.

Children who are born without their upper lateral incisors must have the space maintained where the teeth should be until they are old enough for dental implants.

About 2.5 percent of people in the United States are born with congenitally missing upper lateral incisors, according to information available on DentalResource.org. It is typical for adult teeth to be missing, as opposed to baby teeth. However, a missing baby tooth usually indicates that the adult replacement likely has not formed.

Most young patients complete their treatment at Eisenhuth’s Eagan and Mendota Heights orthodontics practice by the time they are age 14 to 15. But because they still are growing, dental implants cannot be placed until the patient is about 18 years old.

“That leaves us with several years to maintain the space needed for the future implant,” Eisenhuth said. “It’s no secret that teeth like to move, and without adequate retention, the space will close, necessitating future orthodontics when the patient is ready for the implant.”

The Mendota Heights braces expert recommends that a fixed retainer be placed in the patient’s mouth to maintain the space, combined with either a removable retainer with an artificial lateral incisor, or a Maryland Bridge fabricated by the family dentist.

A Maryland bridge consists of a porcelain tooth baked onto a metal framework . The framework includes metal wings on either side of the artificial tooth that are bonded to the tongue side of the teeth adjacent to the space, Eisenhuth said.

Eisenhuth, who also is a Mendota Heights Incognito braces provider, prefers the Maryland Bridge because the patient can remove their retainer to eat lunch at school and still have a lateral incisor in place.

“If we put it in the removable retainer, the patient will have a large space in the front as they eat their lunch,” she said. “The Maryland Bridge also helps lock-in root position, which is crucial for the oral surgeon when the patient is ready for the implant.”

Once the patient stops growing, an oral surgeon can place dental implants into the jawbone to serve as anchors for porcelain crowns to permanently replace the missing teeth. Dental implants today include immediate load options, meaning that once the implant is placed, the oral surgeon can immediately secure the porcelain crown to it.

Traditional dental implants require a healing process of several months to allow the implant to integrate itself into the jawbone through a process called osseointegration.

Once placed, dental implants and porcelain crowns have the look, feel and function of natural teeth, Eisenhuth said.

Following these oral hygiene tips can help move along orthodontic treatment, because healthy teeth adjust more quickly.

EAGAN, ST. PAUL AND MINNEAPOLIS, MINNESOTA – Eagan Orthodontist Dr. Jennifer Eisenhuth recently released a tip sheet on the ways in which good oral hygiene can improve the orthodontic treatment experience and even speed up treatment time.

A question commonly asked of the Eagan braces provider is how long orthodontic treatment takes, so Eisenhuth created the tip sheet for patients to refer to, she said.

“Basically, we want to get the point across to patients that brushing and flossing regularly, maintaining healthy gums and keeping up with routine dental visits absolutely makes a difference when it comes to the length of orthodontic treatment,” said Eisenhuth, who also is an Eagan Incognito braces provider.

The following are Eisenhuth’s oral hygiene tips for patients in orthodontic treatment:

1.	The cleaner the teeth, the less friction.

Clean teeth create less friction, making it possible for teeth to shift into proper alignment easier and quicker during orthodontic treatment.

1.	Gum health can affect treatment.

Red, inflamed gums can slow the movement process and extend treatment time. Good oral hygiene habits are important when it comes to reducing the chances of gum irritation and inflammation.

1.	Brushing and flossing always are important, but especially while in orthodontic treatment.

Brushing, flossing and using a Waterpik help reduce the chances of cavities, gum irritation and inflammation. Orthodontic bands, wires, brackets and rubber bands have direct contact with the teeth, which means they can harbor bits of food, bacteria and plaque.

“We give each of our patients at our Eagan orthodontics practice a Sonicare toothbrush to use in order to keep their teeth clean and healthy,” Eisenhuth said. “We recommend that everyone use some sort of electric toothbrush.”

1.	Take measures to avoid decalcification.

Decalcification is scarring of the enamel that appears as a white spot or area on the teeth. Patients may see decalcification after their braces are removed. Eisenhuth’s Eagan Invisalign patients tend to avoid this problem because they can remove their appliances when they eat, brush and floss.
This problem results from poor oral hygiene, where the acid in plaque sits on the enamel structure long enough that it starts leeching out the calcium from the enamel.

“Decalcification is a scar, meaning that once it has happened, the results cannot be reversed,” said Eisenhuth, an Eagan invisible braces expert.

Fastidious brushing and rinsing with fluoride products often is recommended to help the enamel’s remineralization process to keep teeth strong and resistant to cavities while undergoing orthodontic treatment.

A recent study found that 75 percent of children with an overjet of 6 millimeters or more would have fractures of their front teeth by age 12 if the malocclusion went uncorrected.

ST. PAUL AND MINNEAPOLIS, MINNESOTA – Correcting a child’s overjet- commonly referred to as “buck teeth”- often is a priority among parents when it comes to orthodontic treatment.

As children get older, it can become a priority for them as well, due to issues of self-consciousness.

In addition stopping the ridicule that can come during those awkward adolescent years, there are legitimate health-related reasons for correcting overjet in early childhood, said Minneapolis orthodontist Dr. Jennifer Eisenhuth.

A recent study indicated that 75 percent of children with an overjet of 6 millimeters or more would have fractures of their front teeth by age 12 if the malocclusion went uncorrected, Eisenhuth said.

The American Association of Orthodontists recommends that children be evaluated by an orthodontist by age 7 because many issues are more easily corrected when identified in their early stages, said the Minneapolis braces provider. Since the front teeth also begin to erupt about this time, Eisenhuth also can monitor young patients with overjet and determine the best time to take action to correct the problem.

Overjet can be caused by teeth that overlap or are crowded. Children who suck their fingers or thumbs can cause their top teeth to stick out. Patients who have small lower jaws or lower jaws that are too far back often have upper front teeth that look more prominent.

Correcting Overjet
When discussing methods in which overjet can be corrected, it is important to understand what exactly the term means.

“Lots of people who aren’t in the orthodontic field tend to use overjet and overbite as synonymous terms, but they aren’t,” said the Minneapolis orthodontics expert.

Overjet refers to the horizontal distance between the upper and lower front teeth, Eisenhuth said, while overbite refers to the vertical distance between lower and upper front teeth.
Correcting overjet in children can be done in a variety of ways, depending on the severity of the problem. The needs of each individual patient are taken into consideration when determining the best treatment path, Eisenhuth said.

If the overjet is caused by thumb sucking, the problem can be alleviated by stopping the harmful habit. If at-home methods don’t work, Eisenhuth can install an appliance in the child’s mouth that takes away the pleasant sensation they get from sucking their fingers or thumb.

When crowding is the culprit, braces can correct the crowding issues and guide the upper front teeth into proper position.

Some patients may benefit from growth plates or functional appliances that work to bring the lower jaw forward to close the space between the lower and upper teeth. These appliances can be fixed or removable, depending on the patient’s age, Eisenhuth said. It usually is a good idea to start this type of treatment about age 10 to 12, or before a child begins the growth spurt brought on by puberty, so the child’s facial and skeletal growth can help direct the jaw growth.

“There are some instances where I may recommend beginning treatment at a younger age,” Eisenhuth said. “For example, if a child’s overjet is so severe that the teeth risk being broken or damaged, if the child’s speech is affected, or if the child is being teased and ridiculed at school, we may opt to begin treatment earlier.”

For adults with overjet due to a recessive lower jaw, surgery often is the only answer.

Occasionally, Eisenhuth gets patients who are concerned about aesthetics in their orthodontic treatment. In some isolated cases, Invisalign can be used to correct overjet. However, this isn’t always a viable option, said Eisenhuth, a Minneapolis Invisalign preferred provider.

Most often, the best correction method is done via braces.

This will be the third year that Dr. Jennifer Eisenhuth has sponsored the children's activities at the City of Eagan's July 4th celebration.

EAGAN, INVER GROVE HEIGHTS, ST. PAUL AND MINNEAPOLIS, MINNESOTA – The 2010 Independence Day Funfest celebration in Eagan marked the third year that Eagan and Inver Grove Heights orthodontist Dr. Jennifer Eisenhuth served as a Funfest sponsor.

Funfest is a four-day event that began July 1 with the Miss Eagan Funfest Ambassador Coronation and ended July 4 with the fireworks display.

This year’s event also helped the city celebrate its 150th birthday with a separate parade on July 3. Part of the celebration included bringing back the Children’s Funfest Bike Parade, an old tradition. Other activities included concerts, Old West reenactments, a pancake breakfast and a comedy juggling show.

Eisenhuth’s Eagan and Inver Grove Heights orthodontics practice sponsored the children’s activities, which were held on July 3 this year, according to the Funfest website.

Two years ago, Eisenhuth, an Inver Grove Heights Invisalign specialist, donated items to be passed out to children at the event. Last year, her $1,000 donation sponsored the children’s activities.

“We enjoy taking our children to family and community-oriented events such as this, and we know other families do, too,” said the Eagan and Inver Grove Heights braces provider. “Having plenty of activities to keep the kids busy makes it memorable for them and we wanted to have a role in making that happen. We were happy to be able to do that once again this year.”

The success of community events such as Funfest are dependent upon the generosity of individuals and businesses throughout the community, said Raleigh Seelig prior to last year’s event. Seelig has chaired the July 4th Funfest committee for more than 20 years.

Dr. Jennifer Eisenhuth serves as a sponsor to numerous youth athletic programs each year by offering custom mouth guards to players.

EAGAN, ST. PAUL AND MINNEAPOLIS, MINNESOTA – The Eagan orthodontics practice of Dr. Jennifer Eisenhuth is on board as a sponsor again this year for several youth athletic programs.

Eisenhuth has sponsored local hockey, soccer, lacrosse and football programs for several years by offering custom-fitted mouth guards for players and providing check-ups to ensure players’ mouth guards fit properly.

“Mouth guards are a very important piece of safety equipment for children who play contact sports,” said the Eagan orthodontist.

They help protect against broken or chipped teeth, getting teeth knocked out, and they help protect against bone and root damage, according to the Consumer Guide to Dentistry. Mouth guards also help protect players of all ages from getting more serious injuries including concussions, fractured jaws, neck injuries and cerebral hemorrhages by helping to prevent situations where the lower jaw jams into the upper jaw.

Keeping oral soft tissue away from the teeth helps prevent bruising and cutting the cheeks, tongue and lips, particularly for athletes who also wear orthodontic appliances, the site stated.

“Something that is important for parents of young athletes to remember is that as their children grow, last year’s mouth guard may no longer fit properly to prevent injury,” said Eisenhuth, an Eagan braces expert. “Kids outgrow their skates and uniforms, and they outgrow their mouth guards, too.”

Eisenhuth recommends that parents opt for having a custom mouth guard made for their children, as opposed to buying an over-the-counter mouth guard in a sporting goods store. This is done by having an impression of the mouth taken. Once the mouth guard is fabricated according to the impression, Eisenhuth inspects it to ensure that it fits properly and enables the child to breathe and speak while it is in place.

A proper-fitting mouth guard should remain in place during contact sports, be durable and should be tear-resistant, she said.

Impacted molars can create problems with chewing and can disrupt the stability of the dental arch, not to mention the aesthetic effects it can have on a person's smile.

APPLE VALLEY, ST. PAUL AND MINNEAPOLIS, MINNESOTA – When a child has an impacted permanent tooth, the obvious solution often is simply to pull the baby tooth to make room.

But when it comes to molars, that solution isn’t always the best option, said Apple Valley Orthodontist Dr. Jennifer Eisenhuth.

Impacted first molars generally become evident around age 6 or 7, as those are when the first molars erupt, said Eisenhuth, who treats patients with this problem at her Eagan and Apple Valley orthodontics practice.

What typically occurs is that the first molars erupt tipping forward, causing them to catch on the primary molar in front of it, she said.

Although clinically, Eisenhuth finds that the upper first molars are more likely to impact, lower second molars also can become impacted. It is less common and studies report that it occurs in about three in every 1,000 patients.

It is believed the cause may be due to insufficient skeletal development, namely inadequate arch length, according to a November 2003 article published in the Journal of the American Dental Association.

Another possible cause of impaction is extra room between the roots of the first molar and the crown of the developing second molar, the article stated. This condition may allow the growing second molar to tip in its eruption path.

If the decision is made to remove the primary molar, the space left behind must be held open until that primary molar is actually due to fall out, typically around age 12. When it’s the second molar in question, removing it too soon could mean that the third molar will erupt into the space where the second molar should be.

“A better solution often is to attempt to move back the erupting molar, essentially ‘un-wedging’ the tooth from the primary molar,” said the Apple Valley braces expert.

In cases where extraction is ruled out as the best method of correcting the problem, Eisenhuth sometimes inserts a brass separating band or ligature to allow the molar to correct itself. The band is placed around a tooth toward the front and includes an arm that helps move the molar distally- or back. If this procedure is successful, the molar no longer is impacted, and it will grow in as it should. The primary molar it was wedged against also stays in until it is due to come out on its own.

Another option if the tooth is severely impacted is to have an oral surgeon surgically uncover the molar and upright the tooth and reposition it if necessary to prepare it for proper eruption.

If untreated, impacted molars can create problems with chewing and can disrupt the stability of the dental arch, not to mention the aesthetic effects it can have on a person’s smile.

“Detecting impacted molars is just one more reason why children should be seen by an orthodontist by about age seven as the American Association of Orthodontists recommends,” said Eisenhuth, who also is an Apple Valley Invisalign preferred provider. “Identifying issues such as these early on means they can be treated early and often can take someone off of a surgical path.”

Addressing all eruption and impaction issues when children are young also makes future orthodontic treatment easier, she said.

Supernumerary teeth can create orthodontic problems for patients, but early detection and treatment can lessen the severity of issues, or eliminate the issues altogether.

APPLE VALLEY, ST. PAUL AND MINNEAPOLIS, MINNESOTA – In a perfect world, children are born with 20 primary teeth and 32 permanent teeth.

Sometimes, however, children can be born with supernumerary teeth- a condition known as hyperdontia- meaning they have teeth that exist in addition to the normal series of teeth, said Eagan and Apple Valley Orthodontist Dr. Jennifer Eisenhuth.

Multiple hyperodontia can be related to syndromes and other problems, including Fabry-Anderson syndrome, Gardner’s syndrome, Ehlers-Danlos syndrome, cleidocranial dysostosis or labial palatal cleft, according to a case report on the topic. It is rare for a person to have multiple supernumerary teeth and not have some sort of syndrome.

Supernumerary teeth often get in the way of the normal dentition. The most common supernumerary tooth is a mesiodens, which is a peg-like, poorly formed tooth that occurs between the two front teeth of the upper jaw. In some cases, children may have fourth or even fifth molars that develop behind the third molars as another type of supernumerary teeth, Eisenhuth said.

In a survey of 2,000 children, 0.8 percent had supernumerary teeth present in their primary dentition, while 2.1 percent had them in their secondary dentition, according to a study cited in a Journal of the Canadian Dental Association article.

“In some cases, supernumerary teeth can prevent permanent teeth from erupting properly,” said the Apple Valley braces provider.

Early Detection
Early detection of hyperdontia can lessen the severity of or even prevent future orthodontic problems, said Eisenhuth, who also is an Apple Valley Invisalign provider.

“That’s another reason why it’s good to have children visit an orthodontist by age 7,” she said. “A panoramic X-ray can detect supernumerary teeth and once we know what we’re dealing with, we can create a treatment plan.”

Early detection also means the supernumerary teeth can be removed at the correct age.
“Taking them too early can be difficult for the child or the developing dentition, and taking them too late can harm the roots or crowns of erupting teeth,” Eisenhuth said. “It must be timed well.”

Hyperdontia Treatment
When Eisenhuth sees hyperdontia patients in her Apple Valley orthodontics office, the treatment plan she recommends varies according to the type of supernumerary teeth present and the potential problem they pose. Treatment may include simply extracting the extra teeth.

“In patients where their permanent teeth can’t erupt properly, we often find that removing the supernumerary tooth enables the permanent tooth to erupt without any problems, she said. “In some cases we may use orthodontic treatment to create enough room for the permanent tooth to erupt properly.”

Treatment also could involve extraction followed by orthodontic treatment to create ideal alignment, she said.

In some cases, supernumerary teeth fuse with permanent teeth and correction requires endodontic treatment, according to a case report published by the Journal of Oral Science. Surgical removal of the fused teeth is required because of the tooth’s abnormal structure and width, which can create alignment, crowding and occlusal function issues.

Fused teeth also tend to be more prone to periodontal disease and cavities, according to the JOS article.

Photographs of American Idol contestant Crystal Bowersox show that she appears to have congenitally missing cuspids, a problem commonly addressed by orthodontists.

EAGAN, ST. PAUL AND MINNEAPOLIS, MINNESOTA – Ever since Crystal Bowersox auditioned for American Idol, her teeth- as well as her talent- have generated a great deal of conversation among pop culture bloggers, and Mendota Heights Orthodontist Dr. Jennifer Eisenhuth is using it as an opportunity to shed light on the issue of congenitally missing teeth and teeth whitening.

In photos of Bowersox, it appears that she is missing her cuspids, or canine teeth, said Eisenhuth, whose Eagan and Mendota Heights orthodontics practice treats patients with congenitally missing teeth.

About 20 percent of the population has congenitally missing teeth, according to an article on www.dentalresource.org, a dental health Web site. Most of these cases are missing wisdom teeth or third molars. Less than 3 percent of the American population is born with congenitally missing upper lateral incisors, while slightly more than 3 percent lacks second molars.

The best solution for Bowersox and others like her who are missing their cuspids is to have dental implants, said Eisenhuth, a Mendota Heights braces expert.

Implants with porcelain crowns have the look, feel and sturdiness of natural teeth and can last for quite some time when cared for properly, she said, adding that getting dental implants is fairly simple for youth with healthy bone.

An oral surgeon can’t place implants until the patient stops growing, which is at about age 18. At age 24, Bowersox could be a candidate for implants, provided she has no other underlying health issues, said Eisenhuth who also is an Eagan and Mendota Heights Invisalign provider.

Bloggers also chatted up a storm about Bowersox’s dingy-colored teeth at the beginning of the season, but in subsequent episodes, it is apparent she had them whitened and perhaps had a professional teeth cleaning.

“Teeth whitening is one of the easiest, most economical cosmetic dentistry procedures that can be done,” Eisenhuth said. “We educate our patients on good brushing habits and advise them on the benefits of using whitening toothpaste during treatment, and we recommend that they see their family dentist if they are interested in using whitening gels after treatment.”
Cleaning aligners and other removable mouth appliances is important to eliminating odor and discoloration over time.

EAGAN, ST. PAUL AND MINNEAPOLIS, MINNESOTA – If you wear clear aligners or retainers, you know that keeping them clean and odor-free can be a challenge.

Dr. Jennifer Eisenhuth, a Minneapolis orthodontist, has a couple of tips for keeping Invisalign and other types of clear plastic aligners looking good.

Avoid Toothbrush/Toothpaste
Normally, a toothbrush and toothpaste are great for cleaning a variety of appliances, but not in the case of clear, plastic aligners, said Eisenhuth, a Minneapolis Invisalign provider. The mildly abrasive texture of toothpaste scratches the plastic and changes it from clear to cloudy over time.

It’s also difficult for a toothbrush to reach all of the crevasses in an aligner, which allows plaque and tartar to build.

Bleach
As odd as it may sound, soaking aligners in a bowl of water with a couple of drops of bleach in it can work wonders when it comes to keeping aligners clean and bright.

“The bleach actually works with the materials in the aligner to make them clearer over time,” said the Minneapolis braces expert, adding that it is important to rinse the aligner well after soaking it so the odor and taste of bleach aren’t detected when wearing the aligner.

Cleaners
Denture cleaners are another option for keeping aligners looking their best. There also are some cleaners designed specifically for orthodontic appliances, including Retainer Brite, Ortho Clean and Retainer Splash, just to name a few.

These products kill bacteria and germs, help remove tartar and plaque, and brighten and clean aligners and other types of orthodontic appliances.

It’s important to keep clear aligners and other removable appliances such as night guards, retainers and mouth guards clean, Eisenhuth said. Doing so will prevent odors and bacteria from accumulating.

Correcting facial asymmetry is easier in children than adults. However, there are various treatment methods for patients, regardless of age.

ST. PAUL AND MINNEAPOLIS, MINNESOTA – Skeletal asymmetry of the jaw is one of the most visible facial malformations.

Often, the preferred treatment is orthognathic surgery, but many facial asymmetry issues in children can be corrected or vastly improved with various non-surgical orthodontic treatment methods, provided the issue is identified before the child is finished growing.

As more adults seek orthodontic treatment and facial asymmetry is identified, more inquiries are being made into treatment alternatives to orthognathic surgery, even if it means the end result is not as good as it would have been with surgery, said Dr. Jennifer Eisenhuth, an Eagan and Inver Grove Heights orthodontist.

“I think that for many adults, the asymmetry is something they have dealt with their entire lives, and they view a slight improvement as being better than no improvement at all,” Eisenhuth said. “Many are willing to take the chance for relapse if it means avoiding an invasive surgery. However, that’s seldom the option an orthodontist prefers, and because we know of the excellent results that can be achieved through orthognathic surgery, we tend to seek the most ideal result for our patients.”

Facial skeletal asymmetry is the absence of proportion in the facial features. It can occur naturally, as a result of poor habits, or be the result of trauma, such as a fractured jaw. Skeletal asymmetry should be corrected because it can effect jaw movement, create abnormal wear patterns on the teeth and inhibit a person’s ability to chew properly, said Eisenhuth, who treats Inver Grove Heights orthodontics patients, as well as those from throughout the Twin Cities area.

Facial asymmetry in children can result from many things, including thumb-sucking, cross bites, mouth-breathing, tongue-thrust swallow patterns and cleft palate, Eisenhuth said. In those cases, if caught early enough, Eisenhuth can correct the foundation of the asymmetry and hope the remaining facial structures will follow suit during upcoming growth.

The fact that children have not finished growing works to the advantage of non-surgical treatment. Alterations can be made as the child’s bones continue to grow and further develop.

“Ideally, facial asymmetry is identified and corrected when the patient is young- in the neighborhood of eight to nine years old,” said the Eagan and Inver Grove Heights braces expert. “Correction can be successful if the patient is a bit older- eleven to thirteen years old- but is less likely to be fully corrected. The result will be improved, but compromised from ideal.”

Once a person with facial asymmetry reaches adulthood and growth stops, correcting the malocclusion can be difficult without surgical intervention.

“But it’s not impossible, if the patient is willing to accept results that are improved, yet not ideal,” said Eisenhuth, an Eagan and Inver Grove Heights Invisalign provider.

When Eisenhuth treats adults with facial asymmetry, she first evaluates to determine whether the asymmetry is the result of skeletal or dental causes before a treatment strategy can be planned.

For adults who refuse orthognathic treatment, camouflaging the asymmetry through orthodontic mechanics can be a viable alternative, she said. In select cases, Eisenhuth uses mini-implants to help reposition the teeth and achieve more uniform alignment, which can improve and sometimes correct the asymmetry.

“Even with that, relapse will be rampant,” she said. “So, facial asymmetry in an adult is a tricky road. If the patient is willing to accept a compromised result, some improvement may be possible, but obviously the best answer is orthognathic surgery.”
